The posted compensation range of $36.00 - $42.12 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

As a Supervisor, your role primarily focuses on directing and coordinating all operational, educational, and outreach activities for the Asthma Management Program. Additionally you will provide key fiscal, administrative, and operational support to the department.



Every day, you will actively assist with evaluating program content, managing staff and resources, and striving to improve the health outcomes for individuals with asthma. Additionally you will provide crucial fiscal, administrative, and operational assistance to the department.



To be successful in this role, you will demonstrate strong leadership, communication and interpersonal skills, have a strong understanding of health education principles, particularly related to asthma management, be able to problem-solve, highly organized, and demonstrate fiscal and administrative proficiency.

  • Lead the activities of the Asthma Management Program (AMP)



      • Drives program visibility and meets enrollment goals through targeted outreach efforts.


      • Process referrals and service authorization requests.


      • Oversees the provision of program services.


      • Manages all billing activities.


      • Manages all other fiscal activities.


      • Oversees and coordinates all tasks concerning external databases and portals.


      • Supervises the activities of the Outreach Specialist


      • Ensures compliance with all necessary reporting obligations.
  • Support fiscal, administrative, and operational activities of the Department



      • Manages all financial tracking and reconciliations.


      • Oversees procurement and vendor coordination.


      • Provides general administrative support.


      • Assists with building maintenance.
  • Promote department health related services.



      • Distributes information about services.


      • Represents the Community Wellness Program in community collaborative groups.


      • Participates in health fairs as assigned.
  • Develop and submit monthly, quarterly and annual reports.



      • Prepares and submits professional reports by assigned deadlines that demonstrate progress towards goals and objectives.


      • Demonstrates knowledge of program activities by contributing statistics, successes and challenges when needed for reports and grants.


      • Creates and Utilizes data collection systems to gather and analyze outcome data.
  • Perform other related duties as assigned.



      • Serves as an active member on Hospital committees.


      • Participates in leadership meetings and trainings.

Job Requirements

Required

  • Bachelors Degree or 4 years equivalent work experience., upon hire and
  • 3 years experience in health education, community service, or social service and
  • Basic Life Support - CPR, upon hire and
  • Driver's License: CA, upon hire and
  • Auto Insurance, upon hire


Preferred

  • Previous experience providing training to peers, volunteers, community members and other adults.

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
