The posted compensation range of $29.36 - $43.68 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Are you a passionate Registered Nurse ready to lead and shape the future of community health? LakeWood is seeking a dynamic Community Health Supervisor to oversee and elevate our vital public health programs.

What You'll Do:

As our Community Health Supervisor, you'll be at the forefront of public health, ensuring seamless service delivery and continuous improvement. Your responsibilities will include:

  • Leading our Public Health department and RHC visiting nurse program.
  • Maintaining the highest standards of quality and safety in all services.
  • Guiding financial oversight for personnel and supplies.
  • Developing and implementing policies that align with our mission.
  • Ensuring compliance with State Department of Health standards.
  • Fostering a collaborative and positive work environment.
  • Overseeing risk management and quality improvement initiatives.
  • Engaging with community partners and developing contracts.
  • Mentoring staff and sharing your expertise.
  • Driving emergency preparedness planning and training.
  • Collaborating on community health needs assessments and strategic planning.
  • Managing the departmental budget.

Job Requirements

  • Graduate of an accredited school of professional nursing with current licensure in the State of Minnesota.
  • Certified in Public Health Nursing by the Minnesota Board of Nursing. 
  • RN Licensure in the State of Minnesota
  • BLS
  • Drives License
  • Auto Insurance

CHI LakeWood Health provides long-term care, family practice-clinic, 15-bed critical-access hospital, public health and assisted living services.
