The posted compensation range of $32.30 - $48.05 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

The Swing Bed Program Manager provides oversight of the Swing Bed Program ensuring the effectiveness of the plan of care and the patient's satisfaction. The SBPM engages the necessary disciplines and assesses their compliance with the plan of care to return the patient to their pre-hospitalization level of functioning if possible. The SBPM will also be considered the subject matter expert regarding the discharge plan of the swing bed patient based on the program course of rehabilitation and treatment and/or available post-program resources to continue a path toward wellness.
- Ensures the Swing Bed Program adheres to all federal regulations.
- Interacts and engages with acute health care partners to create and maintain a robust Swing Bed referral pipeline.
- Participates in multidisciplinary rounds.
- Engages with physicians to assess patient’s appropriateness for the Swing Bed program.
- Assesses swing bed patients holistically to ensure appropriate referrals and plans of care while in the program and post-discharge.
- Interacts frequently with swing bed patients to monitor their progress or lack thereof, enabling modification of the plan of care accordingly.
