The posted compensation range of $60.60 - $97.03 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

As our Certified Clinical Pharmacist, Ambulatory Services, you will provide direct patient care in hospital or community-based clinics, working collaboratively with healthcare providers and patients to optimize drug therapy and outcomes.

Every day you will utilize evidence-based standards of care to deliver effective, efficient, and accurate ambulatory pharmaceutical care, ensuring all actions are within your license's scope and compliant with organizational protocols and state regulatory requirements.

To be successful in this role, you will be a Certified Clinical Pharmacist with expertise in ambulatory care, strong clinical judgment, excellent communication and collaboration skills, and a commitment to achieving organizational and patient care goals through optimized drug therapy.

  • Prescribe pharmaceutical agents as outlined in the approved collaborative drug therapy management agreement including initiating, adjusting, or discontinuing drug therapy.
  • Participates in the evaluation and management of diseases and health conditions in collaboration with other health care providers.
  • Order and interpret drug therapyrelated tests.
  • Ensure patients have access to and receive safe, efficacious, and costeffective medication therapy.
  • Work collaboratively to resolve medication access and adherence issues.
  • Provides patients and/or families with focused teaching on safe and effective use of prescribed medications and nonpharmacologic management of chronic health conditions.

    BCPS for Board Certified Pharmacotherapy Specialist
    BCTXP for Solid Organ Transplantation Pharmacy
  • Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies
  • Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
  • Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served and infection control, safety and risk management standards as relates to the ambulatory pharmacy setting

Preferred

  • Graduation from an accredited school of pharmacy
  • Pharm.D. with: Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities, or Completion of a PGY2 Ambulatory Care Pharmacy residency, or Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities
