Responsibilities

The Virtual Nurse is responsible to provide and coordinate care to individuals and families using a holistic approach. The nursing process is used as a structured mechanism for critical thinking and problem solving in providing effective and appropriate care within a safe and confidential environment. The incumbent is responsible for collaborating with members of the healthcare team, personal/professional development, and conducting him/herself professionally. The incumbent will have excellent communication skills, with an overall understanding of the continuum of care needs, safety, quality and patient experience outcomes. The incumbent will be able to effectively and efficiently speak with patients to facilitate progression of care, including education, assessment clinical status and providing interventions that are patient specific partnered with patient/family. The incumbent directly supports the organization’s efforts to enhance the patient experience, promote patient safety and improve risk reductions. 

As a Virtual RN, the Nurse will develop the plan of care and provide comprehensive oversight of the care of the patient remotely across the care continuum. The virtual model will operate the Virtual RN will reside and deliver virtual care to patients through predetermined care inclusion and exclusion criteria. The vRN in partnership with the other clinical team members will be accountable for client care outcomes through the assimilation and application of evidence based practice to design, implement, and evaluate client plans of care in a virtual environment setting. The Virtual RN will work closely and in collaboration with staff, leaders, and executive management to ensure a seamless and high performing approach to the patients they serve. A high degree of professional competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader.

Virtual Nurse Functions/Command Center vRN functions (Acute/Post Acute/Ambulatory): 

  • Collaborates and communicates effectively virtually with physicians, nurses and other health care team members in the provision of clinical care across the continuum 
  • Assesses and reassesses virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history and monitoring and evaluating diagnostic studies as medically indicated. 
  • Interprets available data in order to establish a primary plan of care and intervenes as required. Participates through an interdisciplinary team approach and through multi-dimensional patient care conferences, ongoing patient assessments, and re-evaluation of patient problems or needs to develop a plan of care. 
  • Prioritizes clinical information with critical thinking to escalate patient instability with care plan and medical regimen Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decision-making and provides follow-up to ensure that patient problems are resolved. 
  • Provides direct observation via virtual technology, proactive surveillance of adverse physiological trends, and assurance of appropriate prophylactic strategies, medical chart review, and appropriate reaction to appropriate alerts. Prioritizes and manages multiple electronic devices and software applications in 
  • Coaches and mentors other clinical team members 
  • Virtually educates the patient and families on disease process, procedures. 
  • Works to exceed national benchmarks for quality care delivery, cost containment and length of stay indicators. Maintains patient privacy and confidentiality 
  • Assumes responsibility for personal and professional growth through participation in unit meetings, in-service programs, continuing education programs, and fulfills yearly requirements as stated by policy. 
  • Participates in achieving nursing division goals and supports a shared leadership structure. 
  • Assures appropriate level of understanding, awareness and compliance with all applicable TJC, federal, state and agency laws, regulations, guidelines and professional standards. 
  • Through a defined reporting process, reports clinical, administrative, or technical problems that result in medical errors, unsafe care, or sub-optimal communication between appropriate health care professionals. 
  • Demonstrates work practices consistent with CommonSpirit Health and department-specific safety, security and infection control policies.

Qualifications
  • Current license Registered Nurse 
  • Valid Advanced Life Support (ALS) certification required within 6 months (if applicable) Basic Life Support (BLS) certification required 

  • Exemption for ACLS for telehealth RNs and VRN practiceoutside of the acute care setting

  • Minimum of 3+ years of recent experience as licensed nurse 
  • Knowledge and ability to facilitate patient care across the  continuum
  • Effective communication skills
  • Knowledge of use of EMR 
  • Ability to use multiple technologies; audio/visual
  • RN, Bachelors of Science in Nursing, preferred
  • The virtual nurses' activities around the administrative aspect, rationale, and impact of virtual care. Includes administrative planning, understanding the rationale for telehealth/virtual care and its impact. Understanding the preparation process of telehealth program implementation, and the knowledge that the telehealth nurse will have the more considerable understanding of what it takes to establish and deliver a telehealth program 
  • Competencies around the virtual nurses' activities and interactions with patients/family. The virtual nurses must have the knowledge and skills to maintain consent and confidentiality while interacting with patients/family. Competency around virtual nurses' professional behavior and communication skills while connecting with patients or onsite colleagues; includes making a professionally set background to engender trust 
  • Virtual nurses' activities and interaction with technology. 
  • Skills in the use of technology including, turning it on, connecting to the site/patient managing peripherals e.g., otoscopes, and triaging or responding to alarms or incoming requests 

    onsite. Includes communication and exchange of information, coordination of care with clinical colleagues; e.g., medication management, appropriate referral/connections, and creation of telehealth protocols. 

  • Virtual nurse interactions with peers/colleagues who are onsite. Includes communication and exchange of information, coordination of care with clinical colleagues;  e.g., medication management, appropriate referral/connections, and creation of telehealth protocols.
