The posted compensation range of $58.94 - $89.62 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

As our MDS Coordinator - RN is responsible for program development for specialty Subacute care unit program through education to the community including healthcare providers and managed care organizations. This position acts as the MDS RN Coordinator and has the responsibility for the completion and transmission of MDS for Subacute residents. The MDS Coordinator integrates the program OBRA constituencies thereby improving the continuum of patient care. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support strategic plan and the goals and direction of the quality improvement, process, and activities.
• Supervises MDS completion process from day of admission to day of discharge on all residents. Measurements: MDS, RAPS, Observation.
• Collaborates with physicians, physician groups, hospitals, long term care facilities, Regional Center, and all other government agencies to identify Subacute needs and assist in meeting those needs.
• Accurately identifies patient problems, symptoms and behavioral changes and care plans appropriately. Measurements: Care plan, MDS.
• Reports pertinent patient information to all appropriate licensed and unlicensed personnel. Measurements: Care plan, Interdisciplinary team, Observation.
• Coordinates the revision of the patient care plan to meet the needs of the patient. Measurements: Documentation, Interdisciplinary team.
• Coordinates the weekly interdisciplinary team meeting to discuss resident care plan process. Measurements: Documentation, Observation.
• Liaison between the patient, family member, interdisciplinary team and community to assure that services to meet the patient's needs are initiated and met in accordance with their plans of care and treatment.
• Follows strict confidentiality guidelines in all aspects of job performance
Job Requirements
Required
One (1) year experience in acute, subacute or long term care within 6-12 months.
Current and Active CA, RN licensure
AHA Basic Life Support
Broad knowledge of nursing principles, procedures and practices; ICCU/CCU experience and/or ventilator skills/training a plus.
General knowledge of infection control standards and practices; member and participant in professional nursing organizations.
Excellent communication and interpersonal skills and the ability to work with various publics and interdisciplinary teams.
Preferred
6 months experience as MDS Coordinator in Long Term Care setting
