The posted compensation range of $18.15 - $25.63 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

Assist individuals and communities to adopt healthy behaviors. Conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and
community health. May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs, and provide services such as blood pressure
screening. May collect data to help identify community health needs. Provides housing resources and support to avoid eviction or obtain new stable housing.

  • Advises clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.
  • Acts as a resource to all clients, providers and the community.
  • Attends community meetings and health fairs to understand community issues and build relationships with community members.
  • Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Refers community members to needed health services.
  • Advises clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services.
  • Advocates for individual or community health needs with government agencies or health service providers.
  • Identifies or contacts members of high-risk or otherwise targeted groups, such as members of minority populations, low- income populations, or pregnant women.
  • Identifies the particular health care needs of individuals in a community or target area. Interprets, translates, or provides cultural mediation related to health services or information for community members.
  • Provides one to one coaching to those individuals desiring this service.
  • Coordinates and implements education resources, programs and support services to serve the needs of our community as identified by the Community Health Department.

Job Requirements

Minimum:

  • High School Graduate or High School GED, upon hire.
  • Experience working in the community.
  • Driver's License:NV (AUTO:NV)

Preferred:

  • Bachelors Other upon hire and Teaching experience.
  • Community Housing support experience.
