The posted compensation range of $27.98 - $41.62 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

As our Wraparound Facilitator, you will be a central pillar in empowering individuals and families, guiding them toward resilience and holistic well-being through coordinated, individualized support.
Every day, you will drive the development and implementation of highly individualized care plans by engaging natural supports, community resources, and professional services. You'll ensure these plans are person-centered, strengths-based, and culturally responsive, always aligning with the unique vision and needs of the individual and family. Your work will involve meticulous tracking of progress, proactive problem-solving, and continuous team collaboration.
To be successful in this impactful role, you will combine exceptional interpersonal and communication skills with a profound understanding of Wraparound principles, a solution-focused mindset, and the clarity to navigate complex systems while advocating for the best outcomes.
- Conducts regular home visits to assess client needs and provide crisis intervention as needed. Coordinates the care team including the Child and Family Specialist, Parent Partner and Clinician to ensure that the client's unmet needs are adequately addressed and team members are adhering to the care plan.
- Completes comprehensive assessments as per contract requirements and displaying knowledge of psychopathology and comprehensive evaluation technique. (Master's and PhD level employees only).
- Coordinates the child's Plan of Care (POC) ensuring that all services are provided accordingly and continuously reassesses the needs of the child and family with the team modifying the individualized POC as needed.
- Attends and participates in CFT meetings, and IEP's in order to assist with determining a comprehensive plan for the child/children involved in the referral.
- Facilities and monitors child/family progress toward goal attainment and child safety as per the service plan.
- Provides referrals, advocacy, support, and accompaniment to various social service agencies as needed.
Job Requirements
Required
- Bachelors of Arts and 1-3 years experience providing intensive services.
- A minimum of one year's experience in an academic internship delivering child welfare and mental health services.
- Experience providing services to at-risk children and families through the Department of Mental Health and the Department of Children and Family Services.
- Fire Safety, within 90 Days
Preferred
- Masters Of Arts in social work or psychology and Wraparound experience/training.
