The posted compensation range of $24.00 - $29.69 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

As our Health Information Management Technician, you will be instrumental in maintaining the integrity, accuracy, and accessibility of patient health information, playing a critical role in supporting quality patient care, accurate billing, and regulatory compliance.

Every day you will manage patient health records, including scanning, indexing, archiving, and retrieving medical documentation. You will be expected to ensure the completeness and accuracy of health records, process requests for Protected Health Information (PHI) in compliance with privacy regulations, and assist with coding and abstracting tasks as needed, all while safeguarding patient confidentiality.

To be successful in this role, you must possess meticulous attention to detail, a strong understanding of health information management principles, and an unwavering commitment to data integrity and patient privacy.

  • Answers telephone, route calls, take messages, and provide information to customers promptly and courteously. File/sorts patient medical records on a daily basis in terminal digit order in the proper record location accurately and in a timely manner. Use the medical record indices, which is computerized, to determine the patient's medical records number required for accurate filing/retrieval of medical records and/or portion of unit medical records, verification of patient file number (PFN) etc. Assists and/or provides physicians with necessary information/data for the timely completion of records
  • Communicates effectively utilizing electronic or facsimile request indices to retrieve and/or archive documents to/from off-site storage. Prepares records, files and boxes for off-site storage. Responsible for final verification of all scanned images, reviews each record for missing documents, missing signatures or missing text according to written guidelines. Demonstrates competency in all areas of medical records, especially in the area of medical records completion and discharge analysis. Review assigned work queue(s) daily and assure they meet timely processing of all assignments in the queues.
  • Performs analysis of medical records to ensure completeness of documentation. Review each discharged patient record for incomplete or missing items, missing signatures or missing text according to procedure. Analyzes the technical content of the medical record based upon predefined evaluation criteria. Responsible for identifying the physician to who chart deficiencies will be assigned and enters them into computerized chart tracking system. Provides back up coverage for the prepping and scanning area and assists physicians with record completion on a one-to-one basis.
  • Retrieves medical record and/or dictation file upon physicians routine requests to complete record deficiency utilizing computerized record tracking system. Assembles all medical records in strict chronological order (aka: Book order, 1, 2, 3 etc.) for discharge patients according to approved record order. Responsible for monitoring the completion of the birth certificate worksheet in order to complete the legal birth certificates. Responsible for ensuring that the appropriate physician and parent signatures are completed on the birth certificate. Prepares the legal birth certificate and the courtesy birth announcement provided by the hospital.
  • Forward all completed birth certificates to the Health Department (local registrar) in compliance with state reporting requirements within ten (10) days. Maintain organized, accurate file of every live birth. Assists in completion of birth certificate correction(s) on request received from Vital Statistics (AVSS). Assists in the receipt and processing of fetal monitor strips in accordance with medical records storage guidelines. Responsible for placing copy of PKU results in medical chart in a timely manner. Attend all state required workshops for the purpose of receiving updates on all birth recording guidelines and requirements mandated by the State of California.
  • Process release of information requests to include electronic and paper in a timely manner. Responsible for screening release of information (ROI) authorizations and subpoenas for compliance with regulatory standards prior to releasing information ensuring that the correct/appropriate information is released to the appropriate party. Answer telephone inquires regarding release of patient information, provide current status of requested information and forward requested information to proper requestor in a timely manner. Responsible for monitoring hospital's response to legal requests (subpoenas) for the release of patient information. Opens and directs incoming mail, filing as necessary. Monitor the use of outside copy service, ensuring that the appropriate medical records are available and copied in a timely manner. Review requests for the release of patient information to assure they are completed in accordance with legal requirements. Serve as a resource to other departmental personnel regarding the legal requirements for releasing patient information to other health care providers, fiscal intermediaries and government agencies. Responsible for accepting fees for the release of patient information and monitoring the submittal of those fees to the Accounting Department.

Verifies, assembles, and analyzes medical records according to established standards (JCAHO and department of Health) for chart completion. Provides analyses for inpatients. Accountable for delivery of all discharge patients' records to the medical record department. Supports the timely completion of medical records by the medical staff by maintaining a well-organized record completion area of incomplete and complete medical records for filing and storage. Performs duties in support of the medical center mission to ensure the highest quality of patient care in an economically sound and efficient manner.


Job Requirements

Required:

  • High School Graduate or GED General Studies, upon hire
  • Computer literate

Preferred:

  • General clerical experience MedicalRecord experience and
  • Previous clerical/filingexperience in health care setting
