The posted compensation range of $34.00 - $50.58 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

We are currently seeking a highly skilled and dedicated Patient Care Coordinator to join our dynamic nursing leadership team.

As a Patient Care Coordinator, you will play a critical role in supporting the provision of effective, high-quality patient care across our organization. This leadership position is essential to maintaining clinical excellence, ensuring operational efficiency, and championing a culture of safety and patient-centeredness.

What You'll Do (Key Responsibilities):

  • Clinical Practice & Patient Care Oversight: Actively manage and support clinical practice standards and patient care delivery to ensure optimal patient outcomes and adherence to best practices.
  • Resource Management: Strategically plan, allocate, and manage human, fiscal, and other resources to ensure efficient departmental operations and continuity of care.
  • Staff Development & Mentorship: Contribute to the recruitment, ongoing development, and retention of our talented nursing personnel, fostering a supportive and growth-oriented work environment.
  • Quality & Regulatory Compliance: Ensure strict compliance with all regulatory and professional standards. Actively participate in and champion quality improvement initiatives, striving for continuous enhancement in our services.
  • Patient & Employee Safety: Lead and support robust patient and employee safety programs, promoting a secure and healthy environment for everyone in our facility.
  • Enhancing Patient Experience: Champion efforts to foster and strengthen the patient experience, ensuring every patient receives empathetic, respectful, and personalized care.
  • Interdisciplinary Collaboration: Model and cultivate strong, collaborative relationships with internal teams (physicians, allied health, support staff) and external stakeholders to optimize patient care coordination.
  • Clinical Expert & Leader: Maintain a comprehensive understanding of current clinical practice knowledge and provide expert clinical leadership during shifts.
  • Departmental & House-Wide Leadership: Assume departmental leadership responsibilities in the absence of the department manager/director, and provide critical house-wide leadership as a House Supervisor during evenings, nights, weekends, and/or holidays required.

Job Requirements

Minimum Qualifications:

  • Graduate of an accredited professional nursing education program.
  • Current and unencumbered Minnesota Registered Nurse (RN) license.
  • Basic Life Support (BLS) certification (current).
  • Advanced Cardiovascular Life Support (ACLS) certification required. 
  • Professional Registered Nurse (RN) experience in an acute hospital setting is required.
  • Prior charge nurse experience in a hospital setting is required.

Preferred Qualifications:

  • Clinical leadership or nursing management experience is strongly preferred.
  • Department-specific clinical experience preferred.
