To be successful in this role, you will possess a strong grasp of current pharmacy scope of practice and state/federal regulatory requirements. You will demonstrate exceptional attention to detail, a proactive approach to clinical interventions, and the ability to effectively translate complex clinical data into actionable advice for hospital staff and medical teams.

Responsibilities:

  • Perform daily clinical review and verification of physician orders, monitoring for renal/hepatic function, drug-drug interactions, and administration rates.
  • Utilize pharmacokinetic skills to calculate and monitor dosing, ensuring medication safety and therapeutic efficacy based on approved medical protocols.
  • Intervene in real-time when medication discrepancies are identified, providing evidence-based recommendations for dosage adjustments or therapy changes.
  • Maintain accurate and complete pharmacy records while strictly adhering to state and federal regulatory agency requirements for order processing.
  • Serve as a primary point of contact for client hospital liaisons and internal management, responding to communications with professional clarity and urgency.
  • Engage in continuous professional development to ensure compliance with changing regulatory standards and to support the achievement of organizational clinical goals.

Job Requirements

Required:

  • Graduate of an accredited School of Pharmacy with a PharmD or BS Pharmacy degree.
  • Active, unrestricted Pharmacist license in Minnesota
  • Strong communication skills with the ability to provide clear, timely, and professional recommendations to physicians and nursing staff.

Preferred:

  • Experience in clinical pharmacy practice, preferably within a hospital or telepharmacy setting.
  • Proficiency in electronic medication order processing systems and clinical monitoring software.
