The posted compensation range of $52.02 - $77.38 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

We are seeking a dynamic and experienced Supervisor of Care Coordination to lead our dedicated team. This critical role demands a professional with a strong clinical background and proven leadership abilities, committed to optimizing patient care and ensuring seamless transitions.

As the Supervisor of Care Coordination (CC), you will be a vital leader, directly shaping the delivery of patient-centered care in alignment with the CommonSpirit Health Care Coordination model. You will dynamically support and reinforce strategies across clinical resource management, care progression, discharge planning, patient advocacy, and clinical social work. Under the guidance of the Director or Manager of Care Coordination, you will empower your team to achieve departmental and CommonSpirit Health's strategic goals.

Every day, you will provide direct supervision to CC staff, ensuring they meet and exceed operational performance standards. Your expertise will be crucial in implementing CC policies, procedures, and processes, guiding activities related to progression of care, addressing psycho-social needs, and navigating complex regulatory requirements. You will play a key role in accreditation activities, driving program outcomes and quality metrics, all while fostering an environment dedicated to comprehensive and compassionate patient support.


Job Requirements

What you will bring:

  • Associate's Degree in Nursing (ADN) or Master's Degree in Social Work (MSW), or equivalent combination of education and experience.
  • 1-3 years of relevant experience, including a minimum of 2 years in clinical care coordination.
  • Demonstrated leadership experience, such as a Nursing Team Lead, Charge Nurse, or Supervisory role within an acute care setting.
  • In-depth working knowledge of care coordination principles and discharge planning processes.
  • Ability to obtain Case Manager Certification (CCM) within 24 months of hire.
  • Ability to obtain Accredited Care Manager Certification within 24 months of hire.

Preferred Qualification:

  • Bachelor's Degree in Nursing (BSN).
