The posted compensation range of $47.29 - $70.35 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. About the Role:

As a Supervisor of Care Coordination, you will be instrumental in the day-to-day operations and strategic execution of our CommonSpirit Health Care Coordination model. Reporting to the Director and/or Manager of Care Coordination, you will lead a team of dedicated Care Coordinators, empowering them to deliver exceptional service in clinical resource management, progression of care, discharge planning, patient advocacy, and clinical social work.

This is more than just a supervisory role; it's an opportunity to be a catalyst for change, driving continuous improvement and ensuring our care coordination services exceed operational performance standards. You will be at the forefront of implementing policies, procedures, and best practices, guiding your team through complex psycho-social needs and regulatory requirements to achieve outstanding program outcomes and quality metrics.

What You'll Do:

  • Lead and Empower: Provide direct daily supervision, coaching, counseling, and performance management to your care coordination team, fostering a supportive and high-performing environment.
  • Drive Excellence: Reinforce and implement care coordination programs and strategies that optimize patient care, maximize resource utilization, and contribute to superior quality outcomes across CommonSpirit Health.
  • Ensure Compliance: Uphold the highest standards by ensuring strict adherence to all relevant healthcare regulations, accreditation mandates, and organizational policies.
  • Operational Mastery: Support and contribute to the efficient daily operations of the care coordination department.
  • Seamless Collaboration: Forge strong partnerships with internal and external stakeholders to orchestrate patient care and facilitate clear, timely communication throughout the care continuum.
  • Develop and Grow: Train and evaluate care coordination staff, ensuring they are proficient in system initiatives, possess essential skills and knowledge, and complete all required education.
  • Champion for Patients: Act as a steadfast advocate for patients' needs and rights, ensuring they receive personalized and appropriate care and unwavering support at every stage of their healthcare journey.
  • Effective Communication: Maintain open and effective communication channels with senior leadership, clinical staff, and external partners to ensure efficient care coordination operations.
  • Collaborative Innovation: Actively participate in and drive multidisciplinary teams and committees, addressing complex patient care challenges and developing integrated, holistic plans of care.
  • Continuous Improvement: Monitor established goals and Key Performance Indicators (KPIs) to guide and implement strategies for ongoing improvement.
  • Strategic Partnership: Collaborate with Utilization Management Hub leaders and staff to identify opportunities for enhancement and develop actionable plans to address any gaps.
  • Build the Best Team: Play an active role in recruitment, performance management, coaching, mentoring, training, and professional development of care coordination staff.

Job Requirements

What You'll Bring:

Required Education and Experience:

  • Associate's Degree in Nursing (e.g. ADN) or Master's in Social Work (MSW) or equivalent education and experience.
  • Minimum of two (2) years of clinical care coordination experience (Utilization Management, Denial Management, Care Coordination), OR two (2) years of acute nursing team lead, charge, or supervisory experience with working knowledge of care coordination and discharge planning.

Preferred Education and Experience:

  • Bachelor's degree in Nursing, Health Care Administration, or a related clinical field.
  • Understanding of performance improvement activities.

Required Licensure and Certifications:

  • Current, unrestricted Registered Nurse (RN) license in the state of practice, OR
  • Clinical Social Worker (SW) license (as required by state law, referring to Chart A below) OR
  • Initial Social Work license (as required by state law, referring to Chart B below) with a commitment to completing clinical licensure within three (3) years
