The posted compensation range of $32.38 - $48.17 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

As our Coordinator of Strategic Partnerships and Care Navigation will be responsible for coordinating the plans for monitoring and improving the coordination of care and related activities between strategic partners, joint ventures and CommonSpirit aligned specialist services, while prioritizing humankindness and patient-centered care. The Coordinator will responsible for reporting and analyzing trends related to the performance of the portfolio of strategic partnerships who provide a level of concierge-style services to key partners across CommonSpirit Health regions, for healthcare services. Serves as a day-to-day partner in analysis of patient and provider relations data to ensure timely communication paths remain available to coordinate with nursing and allied health support staff to coordinate and assist in the ongoing implementation of process systems to ensure patient needs are met. The Coordinator will assist in the development of standard work to facilitate and coordinate medical records transmission, pre-visit paperwork, and insurance authorization and eligibility process for strategic partnership.
Practices to support service levels enhancements and long-term financial plans. In support of the Manager of Strategic Partnerships, will carry responsibility to ensure the alignment strategy for operational transformation in delivering in-network utilization, often by collaborating with leaders to assess how to create new ways to drive effective care and patient experience. The Coordinator is responsible and accountable to support and steward relationships with sales and marketing teams to create outreach and positioning opportunities to a variety of internal and external stakeholders. Functions as the liaison between CommonSpirit Health and strategic partners for effective care coordination and navigation.
- Responsible for the stewardship of seamless direct patient care coordination, guidance, and education, often serving as the first point of contact for strategic partners within a market of the Physician Enterprise. Supports the implementation of strategic growth initiatives, as set by the strategy of the Physician Enterprise to ensure alignment with CommonSpirit Health’s mission and strategic objectives. (50%)
- Responsible for coordinating activities and standard work in relation to the strategic portfolio of joint venture partnerships. Responsible to deliver data and communications that will enhance the development, transformation of process improvement mechanisms to support growth within a market. (25%)
- Supports the implementation cycle of new joint venture and care navigation partnerships and projects, including, but not limited to the documentation and scope of current state and future state requirements, while maintaining and delivering on standard work to create future optimization opportunities.
- Supports project planning and deployment efforts for new and exiting partnerships, while serving as a subject matter expert to interface with Connection Center, Clinic Operations and strategic partner Leadership related to operationalizing roll out. (25%)
- Responsible for managing and maintaining access protocols and pathway processes for partners and affiliated groups, including communication and implementation plans.
- Assists in ensuring policies, processes and quality initiatives related to departmental growth and improvement metrics are practiced and operating effectively for stakeholders.

Job Requirements
Required
- Bachelors Degree in Healthcare Administration, Business Administration, Communications, or related field
- Three (3) years of relevant business and field experience, preferably in a large multisite organization
- Experience managing Provider relations
- Strong analytical skills specifically with electronic health record systems and various communication systems.
- Proactive thinker with a focus on continuous improvement, experienced in analytical and critical-thinking skills.
- Possesses strong analytical skills and ability to conduct root-cause analysis on complex subjects and translate findings into appropriate action
Preferred
- Three (3) years experience with a strong functional understanding of acute, ambulatory, and billing operations in a large multi-site healthcare organization
- Oncology experience strongly preferred
- Program management experience in a healthcare environment
