The posted compensation range of $35.62 - $52.99 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. As our Care Coordination Masters Social Worker you will be a vital contributor to patient well-being, responsible for delivering crucial social work assessments and interventions to hospitalized and emergency department patients. This role is central to crisis management, high-risk patient screening, and facilitating effective communication between patients, families, and healthcare teams to ensure seamless transitions of care and appropriate resource utilization.
Every day you will engage in diverse responsibilities, including psycho-social assessments, counseling support, and identification of vulnerable patient populations (e.g., abuse, neglect, trafficking). You will adeptly manage referrals for complex psychosocial needs, conduct patient/family care conferences, and educate on health management, bioethics, and community resources including mental health, palliative, and hospice care. This role demands proactive engagement with community agencies and a commitment to upholding organizational values and regulatory compliance through meticulous documentation and continuous process improvement.
To be successful in this role, you will hold a Master's degree in Social Work from an accredited program, coupled with a minimum of one year of post-MSW experience or relevant clinical/medical social work internship. Preferred candidates will bring at least three years of post-MSW healthcare experience. You demonstrate exceptional customer service skills, strong interpersonal and written communication, and proven analytical and problem-solving abilities. A high degree of detail orientation, ability to manage multiple tasks, and effective collaboration within an interdisciplinary team are essential for navigating this fast-paced and impactful environment.
Job Requirements
Required
- 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
- LMSW license from the AZ Board of Behavioral Health Examiners
- Master's Degree from a School of Social work accredited by the Council of Social Work Education
- Ability to work independently as needed with a high degree of detail orientation
- Ability to work efficiently in a fast-paced environment with changing priorities
Ability to work collaboratively with an interdisciplinary care team
Preferred
- Minimum 3-Year Post-MSW healthcare experience
