The posted compensation range of $26.76 - $39.81 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. As our Program Manager, Community Health, you will lead and drive strategic initiatives designed to improve community health outcomes and foster wellness. You will be the key liaison between the organization and the community, responsible for implementing health programs that address critical needs such as diabetes, obesity, and preventative care.
Every day you will coordinate, implement, and evaluate community health education resources and support services to meet the identified needs of the community. You will engage directly with individuals, families, and community groups, providing one-on-one coaching and health education while collaborating with clinical providers to ensure that all messaging and care are culturally and linguistically appropriate.
To be successful in this role, you will possess a passion for health equity and a strong background in community outreach or public health program management. A Bachelor’s degree in a relevant field is preferred, though relevant experience is highly valued.
- The Community Health Program Manager provides strategic leadership, oversight, and coordination of
community health programs designed to address the priority health needs identified through the Community - Health Needs Assessment (CHNA). This position is responsible for planning, implementing, evaluating, and
continuously improving community-based initiatives that promote health equity, improve health outcomes, and address social drivers of health. - The Program Manager leads and supervises a team of Community Health Workers, providing guidance, coaching, performance management, and professional development to ensure high-quality, culturally responsive services.
- The role works collaboratively with multidisciplinary teams, including clinical providers, behavioral health, care management, quality improvement, and community partners, to develop integrated approaches that meet the needs of the populations served.

- High School Diploma/GED
- Certificate Other Community Health Worker
- Two years of management/leadership experience
- Knowledge of community health, population health, public health principles, and health promotion strategies.
Preferred
- Associates Degree
