The posted compensation range of $32.38 - $48.17 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

As our Community Health Patient Navigator, you will be responsible for assisting patients and their support team to navigate and access community services, other resources, and adopt healthy behaviors.
Every day, you will support the healthcare system providers and staff through an integrated approach to care management and community outreach. You will provide social support, informal counseling, and advocate for the health needs of individuals and their community. Community outreach and collaboration with community agencies are essential to this role. You will also participate in the process of collecting and maintaining health-related social needs screening data.
To be successful in your role, you will strategically assist patients and their support teams in navigating and accessing community services, resources, and healthy behaviors, thereby enhancing overall well-being. You will demonstrate exceptional proficiency in providing social support, informal counseling, and advocacy for health needs, meticulously collecting social needs screening data, and fostering strong community outreach and collaboration to create an integrated approach to care management.
- Promote Routine Testing and Treatment: Facilitate routine opt-out testing and treatment for Syphilis, HIV and HCV, within the emergency department (ED)
- Advocate for Cultural Change: Drive efforts to normalize routine opt-out testing by creating awareness, displaying signage, and distributing outreach materials in the ED.
- Build Community Connections: Establish and maintain relationships with community providers to ensure smooth patient referrals and comprehensive care pathways.
- Increase Accessibility: Share contact information widely and respond promptly to inquiries from ED staff, community providers, and patients.
- Educate and Empower Patients: Provide patients with information on select STI effects, treatment options, and chronic disease management while coaching them on preventive practices and harm reduction using motivational interviewing techniques.
- Strengthen Follow-Up Processes: Collaborate with the Clinical Champion to design and implement a follow-up system to ensure patient notification, treatment, and continuity of care.
Job Requirements
- High school diploma or GED.
- Bachelor’s degree in Public Health, Health Sciences, or a related field preferred.
- Two years of experience in community health education or healthcare, working with diverse and underserved populations preferred.
- Ability to establish rapport with diverse groups, including the LGBTQ community.
- Bilingual (Spanish) skills preferred.
