The posted compensation range of $45.06 - $74.35 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

As our RN Case Manager, you will strategically coordinate the care of hospitalized patients, optimizing resource utilization and meticulously planning for safe and timely discharges that meet individual patient needs, all while upholding the highest standards of professional nursing.

Every day you will collaborate closely with a multidisciplinary team, including social workers, providers, utilization review specialists, and ancillary departments. Through this collaboration, and the expert application of care guidelines, you will actively ensure patient progression along established clinical pathways. A critical aspect of your role involves creating a seamless and safe transition for patients into appropriate post-acute settings.

To be successful in this role you will posses exceptional clinical assessment skills, possess a deep understanding of care coordination and discharge planning, and exhibit strong communication, negotiation, and critical thinking abilities to navigate complex patient needs and healthcare systems efficiently.


Job Requirements

Required

  • Bachelors of Science in Nursing
  • Minimum of three (3) years of experience in care coordination, care management, discharge planning, or relevant/related experience
  • Current Washington State Registered Nurse License

Preferred

  • Master's degree in Nursing
