The posted compensation range of $29.44 - $43.79 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

As a Case Manager, you will be a pivotal advocate, orchestrating comprehensive care plans and empowering patients and their families for successful transitions and sustained well-being.
Every day, you will expertly collaborate with physicians, RNs, departments, and community resources to identify and remove care barriers, develop personalized discharge plans, and ensure medical necessity, all while building trust through compassionate, critical thinking.
To be successful in this role, you will combine acute creative thinking and robust critical assessment skills with a deep commitment to patient advocacy, ensuring tailored solutions and optimal outcomes for every individual.
What you'll do...
- Analytical Assessment: Systematically gathers information to identify complex patient needs, recognizes interdependencies among issues, and incorporates interdisciplinary input for evidence-based decision-making.'
- Comprehensive Care Coordination: Manages high-risk care plans from pre-admission through post-discharge. Coordinates interdisciplinary teams to ensure seamless service delivery and resource allocation for complex, multi-need patients.
- Clinical Counseling & Support: Interviews patients and families to evaluate social, emotional, and economic factors; provides the care team with actionable recommendations for the patient’s treatment plan.
- Decisive Problem-Solving: Exercises sound clinical judgment to make timely, informed decisions, even in uncertain or high-pressure situations.
Utilization & Quality Review: Consistently evaluates the appropriateness of care levels, diagnostic testing, and clinical procedures to mitigate risk and ensure high-quality outcomes. - Resource Management: Monitors admission and continued-stay criteria against regulatory standards (e.g., Medicare, PRO) and implements communication strategies to optimize resource utilization.
Job Requirements
Required
- Bachelor’s degree in Nursing (BSN), Social Work (BSW), or a closely related field.
- Must hold a current, unencumbered license as a Registered Nurse (RN) or Licensed Social Worker (LSW) in the State of North Dakota.
Preferred Qualifications:
- Prior experience in case management.
- Background in acute care settings as a nurse or social worker.
